Please note this is a parental consent form and not a medical history/intake form. I’ve found that having a parent fill out a standard intake form for their child works best (click to purchase an intake form template.) Children age 11 and up can also fill out a standard intake form. Young children have an easier time filling out a ‘How Are You?’ form.
